We ask you to pay for services as they are rendered. We will file insurance claims at no additional cost to you. We also ask you to provide us with current and accurate information about your insurance coverage. We are a participating provider for Medicare and Medicaid. We participate in various HMO and PPO plans. All eligible patients are expected to pay co-payments at the time of service. We accept cash, checks, MasterCard and VISA.

Hospitalized patients are asked to sign insurance claim forms before being discharged. You will receive a statement of physician fees from our office, separate from hospital billing. We promptly file your insurance claim upon discharge.

Please remember: Insurance is a way to reimburse you for fees due to the doctor. You are fully responsible for the total amount of the fee for services rendered.
