Appointments

Office hours are Monday through Friday, 9 a.m. to 5 p.m.  We can schedule office visits at our satellite offices in Dunn, Smithfield, Wake Forest, and Cary.  Special scheduling can accommodate unusual circumstances.  We call to confirm each appointment.  Please give us 24 hours notice if you must cancel an appointment so we can free the time for another patient.  We will be happy to reschedule you at a more convenient time.

New patients to Wake Nephrology Associates are scheduled by physician referral.  Our New Patient Referral Form is located under FORMS.  Once we receive the New Patient Referral Form along with the requested records, as outlined on the form, we will contact the patient directly to schedule the new patient appointment.  We will notify the referring physician once the appointment is made or if we are unsuccessful at contacting the patient. 

Follow up appointments may be scheduled by calling our office at (919) 876-7807.

What to Bring to Your First Visit

  • New patient materials including Patient Registration Form, Notice of Privacy Practices, Consent Signature, and Health History form.  We will mail these to you in advance of your appointment, or they can be printed directly off our web site from the FORMs tab.  Please complete these in advance or arrive 15 minutes early to do so.
  • Medication bottles with you or a complete medication list (including all prescriptions and over-the-counter medications with doses and how often you take them).
  • Insurance cards.
  • Photo-ID.
  • Co-pay if applicable.
  • Come prepared to give a urine sample to evaluate for protein, blood, etc.

Accessible To You 24 Hours a Day

One of our physicians is always on call to respond to emergencies - 24 hours a day, 365 days a year. Please call our office if an emergency arises. If the office is closed, our telephone answering service will relay information from you to the physician on call. He will return the call promptly, discuss the problem with you, and make arrangements for emergency care if necessary. It is unlikely, but if you are not contacted within 30 minutes, please call again.


Hospitalization

Hospitalization can cause considerable anxiety. We will explain the cause and treatment of your condition as well as the risks and benefits of each procedure ordered in the hospital. If at any time your status or illness is unclear to you, contact us. We will make every effort to answer your questions.

Your primary physician may request a nephrology consultation while you are hospitalized for another condition. In that case, our staff will visit you, as needed, to complete recommendation for further diagnosis and treatment.


Medications

Please bring your medicine to the office when you come for your appointment.

There are two (2) methods to request refills:

  • Our office has the ability to e-prescribe refills.  Have your pharmacy e-prescribe your refill request directly to our office.
  • Pharmacies can fax the refill request to (919) 876-8823.
